When you interact with an object, npc or item, you almost never click once if you are using the first option.
You spam click a bit when you use the first option of the menu most of the time.
This should be integrated into the bot?

Are you saying that humans spam click when they interact with stuff?

Hmm i dont support that idea tbh. There are too many things that can go wrong when the bot spam clicks while interacting.
Spam clicking is something the bot author should implement instead

I don't mean extreme spam, maybe +2-7 clicks max?

    You are doing it subconsciously

"first option of the menu" = left clicking?

Forcing spam clicks would just contribute to a more bot-like behaviour, though.

A deviation in playstyle, such as clicking (a huge part), would almost definitely get you banned. That's why I think this method could work for new accounts, but at the same time fail for the vast majority of high lvl accounts.
