I think this is one of the ones where the issue occurred on Sunday the 22nd. I might have helped address the issue since then and I've made sure to begin the bot on top of mount karuulm and allowed the bot to navigate to Hydra. I was starting the bot right outside the instance when these issues were happening. Maybe that has something to do with it?

Ah okay. Good news is it's been running well when I start the bot on top of Karuulm, so if it happens again I'll pull the log for the day it happens. Thanks for looking into it!

Okay it's happening now back-to-back instances. This log will show where it occurs, specifically starting at 16:43:17,00:19:41 within the log. The bot can't seem to get flame phase hydra onto the vent and then it teles out.

I see. Is it only when you fail the lightning phase one down?

Actually, I hadn't noticed that, but that would make sense. I output enough DPS when I do the fight on my own to where I don't experience a second lightning phase. It's really strange because the bot goes to the correct tile to drag flame phase hydra over the vent, but hydra doesn't quite stand over it. I'll need to keep an eye out for that.
